WAIVER - CONSENT & RELEASE

Please note this waiver will be kept on file for future events sponsored by Victory Family Church.

THIS WAIVER APPLIES TO THE ABOVE LISTED CHILD(ren):

I, the undersigned, am the parent or legal guardian, named above, of the child(ren) or youth ("Child") named above.

This is a binding legal agreement; therefore clarify any questions or concerns before accepting/submitting the form. As a Participant in camps, events, programs, and activities organized, operated or conducted by Victory Family Church, the undersigned, being the Participant and the Parent/Guardian of the Participant (collectively the “Parties”) acknowledge and agree to the following terms:

I, being the parent or legal guardian of this/these child(ren), have been informed of the above activity sponsored by Victory Family Church and hereby give my consent for my minor child(ren) to participate in this activity.

By consenting, I understand that in the event that medical treatment is required, every effort will be made to contact me. If I cannot be reached, I hereby authorize Victory Family Church, its staff, and event volunteers, as an agent for myself to consent for medical treatment by Hospital Medical Staff, EMT, and other medical professionals. This form will be surrendered to the medical staff so they may be aware of any known medical issues. This authorization is effective from the date of signing this form. I understand that my child and I are responsible for knowing the rules and regulations made by the church and sponsors of this activity. I understand that all reasonable safety precautions will be taken by the leaders of this activity, and that the possibility of an unforeseen hazard does exist. I do hereby release, forever discharge and agree to hold harmless Victory Family Church, its leaders, employees, and volunteer staff from any and all liability, claims or demands for personal injury, sickness or death.

In consideration of the Organization allowing the Participant to participate, the Parties agree:

a) To freely accept and fully assume all such risks, dangers and hazards and possibility of personal injury, death, property damage,expense and related loss, including loss of income, resulting from participating in events/activities hosted by Victory Family Church.

b) To forever release the Organization (Victory Family Church) from any and all liability for any and all claims, demands, actions and costs that might arise out of the Participant’s participation in events/activities hosted by Victory Family Church, or from the physical risks associated with same.

By accepting, you release Victory Family Church, its staff, members, adherents, and otherwise attached from all liability, and responsibility for property, belongings, and self at church functions and the functions of its ministries. By consenting you agree to the expected Code of Conduct (behave, have manners, respect adults and authority, stay with the group, obey the rules, etc).

By selecting "Accept", you are agreeing that if disciplinary measures are needed (i.e. sitting out during the event, missing the event completely, or being sent home with a parent) they are understood by both the parent(s) and/or guardian(s) and the child.

I have read, understand and release Victory Family Church for any liability and/or claims and accept all risks in participating.

I, being the parent or legal guardian of this child, have been informed of the above activity sponsored by Victory Family Church and hereby give my consent for my minor child(ren) to participate in this activity. I have fully informed myself of the contents of this PARENTAL CONSENT AND RELEASE FOR ALL ACTIVITIES by reading it before I signed it. I warrant that I possess all the rights, powers, and privileges of a parent or legal guardian necessary to execute this document with binding legal effect. I UNDERSTAND THAT BY SELECTING

MEDIA WAIVER: I understand that during the course of this activity, pictures and video of my child may be taken. I hereby release Victory Family Church to use such print and video media at their discretion for advertising and promotion and during any event pertaining to Victory Family Church. Names will never be published unless written consent is given by the parent AND child.
