Dear Parents

Victory Family Church of Swift Current will be promoting SK Victory Youth BootCAMP on our website, Facebook Page, and our church APP. All throughout the weekend, there will be photographers and videographers getting great shots of the amazing time of ministry & fun to be had at BootCAMP.

Please fill out the form below indicating whether you approve or deny your child having their picture/video taken and used to promote SK Victory Youth BootCAMP.

NOTE: must be completed by a parent or guardian.

YOUTH LEADERS: if you are registering a group, please email Lori Peters for a PDF form that must be signed by each parent or guardian.
They can be scanned and sent back, or brought to BOOT CAMP. Email: victorybootcamp@sasktel.net

Pre-orders will be available for pick up at the SEW LOVE TUCK SHOP.

PAYMENT INFORMATION

The cost for each camper is $135 (after May 12 it is $160)
Youth leaders cost will remain at $135.

Campers will not be considered registered until payment is received.

1. Fill out all registration details above for each camper.
2. Send payment for all campers you are responsible for registering (# of campers x $135).

Please indicate in the memo of the e-transfer or the note on the online payment link which camper(s) you are paying for.
